Oliver McGowan, 18, who had epilepsy, cerebral palsy and autism, was given antipsychotic medication. McGowan was admitted to Southmead hospital in Bristol on 22 October 2016 after having a seizure that did not subside when his normal medicine was administered.

The inquest has concluded with the coroner finding her death was caused by a lack of oxygen in the body (hypoxia) related to the anti-psychotic drug Clozapine, causing sudden heart failure

Rebecca Riley died age 4 On Dec. 13, 2006, police responded to a 911 call and found a little girl lying dead on the floor next to her parents' bed. The autopsy revealed that she had died from an overdose of psychiatric drugs. Rebecca Riley was being treated for bipolar disorder, or manic depression, even though she was just four years old.

Jonathan Malia: 24-year-old Jonathan Malia who died on 17 January 2013 under psychiatric. Jonathan was admitted as a voluntary patient at the Queen Elizabeth Hospital in Selly Oak on 4 January 2013 before first being transferred to Meadow Croft Hospital in Winson Green and then to the Cygnet Hospital in Stevenage, Hertfordshire.
After reportedly collapsing he was taken to Lister Hospital in Stevenage where he tragically died.

Julia MacPherson, 54, died while under the care of Oxleas NHS Foundation Trust in May 2016 after being put on a trial of clozapine, even though she did not have psychosis and begged not to be put on the medication.

John Eric Kauffman, 41, March 27, 2006 “He was found dead on March 27 at his apartment in Decatur, Ga., just outside Atlanta.
An autopsy showed that the 41-year-old Mr. Kauffman, who was 5 feet 10 inches, weighed 259 pounds when he died. His mother believes that the weight he gained while on Zyprexa contributed to the heart disease that killed him.”

Eric Layne, Died age 29, January 2008 Eric Layne died at home in West Virginia in January 2008 while his wife was pregnant with their second child, Jubilee, who accompanied her mother to yesterday's hearing.
"I am the widow of Sgt. Eric Layne. I found my husband dead when our son, Shamus, was 17 months old," Janette Layne told the committee between her tears.
"In summer of 2007, he began taking Klonopin and Seroquel.
